Qualifications

• Active Colorado Physical Therapist license (required).

• Home health or geriatrics experience preferred.

Responsibilities

• Perform comprehensive in-home evaluations to assess mobility, strength, safety, balance, and functional limitations.
• Develop individualized Plans of Care tailored to each patient’s goals.
• Document all assessments and care plans in the EMR.
