Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to help patients regain strength and independence in their homes. Reporting to the Clinical Manager, you will implement the plan created by the Physical Therapist and work with a collaborative care team.

The role requires a current state PTA license, CPR, and a valid driver’s license with reliable transportation. The position offers a 40-hour work week with a pay range of $64,000–$87,500 annually.

Qualifications

• Licensed physical therapy assistant with a current state license.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Knowledge of geriatric physiological changes.
• Fluent in English: read, write and speak.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes as developed by the Physical Therapist.
• Perform non-complex manual therapy exercises, massage, traction, heat, cold, water, and electrical modalities.
• Document client records and submit clinical documentation on time.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment issues, supply shortages, or staff changes.
• Apply infection control and universal precautions in care activities.
