• Deliver hands-on therapy in the patient's home using evidence-based techniques and assistive devices.
• Educate patients and families on exercises and safe practices between visits.
• Track outcomes and adjust plans of care to ensure effective results.
• Collaborate with interdisciplinary team to coordinate care and transitions between levels of service.
• Document visits, treatments, and progress per agency standards and home care regulations.
• Maintain reliable and timely attendance to ensure continuity of care.
