Job summary

Live Well Home Care seeks compassionate in-home care professionals to support clients in their own homes and communities, helping with daily living activities and recovery.

You will assist with bathing, dressing, mobility, medication reminders, meal prep, housekeeping, transportation, and companionship, ensuring safety and dignity.

Work closely with Branch Manager and Care Team to follow care plans, document progress, and provide respite care to families.

Qualifications

• Provide in-home personal care as per the care plan.
• Assist with activities of daily living and home management.
• Follow agency policies and care plans to support client safety.

Responsibilities

• Assist with activities of daily living and personal care including ambulation.
• Bathing.
• Dressing.
• Exercise.
• Feeding.
• Hair care.
• Medication reminding.
• Mouth care.
• Nail care.
• Positioning.
• Shaving.
• Skin care.
• Toileting.
• Transferring.
• Ensure client’s safety and security by supervising the home environment.
• Meal planning and routine housekeeping per care plan.
• Provide companionship and encourage activities that stimulate the mind.
• Provides respite care for families per care plans.
• Perform errands and essential shopping with supervision as per care plan.
• Assist with diet plan as identified in the care plan.
• Escort clients to medical facilities, errands, and outings as specified.
• Assist clients with communication by writing or typing correspondence.
• Collaborate with Client Care Team and Branch Manager.
