Job summary

Northshore Home Health Care is seeking a compassionate Home Care / Personal Support Worker to provide personal care and basic nursing support to clients in their homes according to each client’s plan of care. You will support daily living needs while promoting comfort, safety, and independence.

Responsibilities include assisting with hygiene, mobility, meals, light housekeeping, companionship, documenting care, and reporting changes to the agency.

Qualifications

• Ability to follow individualized care plans accurately.
• Strong communication and compassionate, client-centered approach.
• Basic understanding of safe mobility and daily living assistance.
• Reliable documentation and appropriate escalation of concerns.
• Medication administration is not permitted.

Responsibilities

• Assist with hygiene, bathing, dressing, and other personal care tasks
• Help with mobility and support prescribed exercises
• Assist with meals and basic nutrition needs
• Offer companionship and promote wellbeing
• Perform light housekeeping tasks related to client care
• Document care provided and maintain accurate records
• Report any changes in condition, concerns, or safety issues to the agency
