Live Well Home Care is seeking caregivers to provide in-home personal care and support for individuals needing assistance due to age, illness, or disability. Duties include ADLs, meal planning, housekeeping, companionship, and respite care, with supervision by the Branch Manager.

Responsibilities cover safety, transportation assistance, medication reminders, and collaboration with the Client Care Team to ensure compassionate, compliant service in home environments.

Responsabilidades

• Assist with activities of daily living and personal care including ambulation, bathing, dressing, exercise, feeding, hair care, mouth care, nail care, skin care, toileting, transferring
• Ensure client’s safety and security by supervising the home environment
• Teach/perform meal planning and preparation, routine housekeeping and laundry
• Provide companionship with social interactions, emotional reassurance, and encouragement of activities
• Provide respite care for families per care plans
• Perform/assist with essential shopping/errands, handling client money under care plan and Branch Manager supervision
• Assist clients with following a written diet plan and reinforce diet maintenance per care plan
• Escort clients to medical facilities, errands, shopping, and outings as specified in the care plan
• Assist clients with communication by writing or typing correspondence
• Participate with Client Care Team by providing input and suggestions
• Ensure service is delivered per relevant policies and practices
• Monitor supplies and resources
• Follow the written care plan
• Carry out duties as assigned by the Branch Manager
• Observe clients for unsafe conditions and report to Branch Manager
