Job summary

First Steps Nursing and Therapy is seeking a licensed Occupational Therapist to provide in-home therapy to children in San Antonio, Texas and nearby areas. The OT will evaluate and treat patients in the home under physician orders, coordinating with families and healthcare teams.

Responsibilities include planning activity-based therapy, documenting progress, maintaining patient records, coordinating visits, and contributing to discharge planning and staff development.

Qualifications

• Licensed Occupational Therapist in Texas (OT).
• 1+ year of work experience preferred.
• Pediatric and home health experience preferred (not required).
• Excellent communication and interpersonal skills to interact with patients, families, and healthcare teams.
• Familiarity with Kantime and EVV systems.

Responsibilities

• Conduct evaluations and therapy in the patient’s home.
• Plan activities and provide occupational therapy services.
• Document treatments and patient responses weekly in records.
• Maintain complete patient records and progress notes.
• Utilize community resources and coordinate referrals.
• Attend case conferences as requested.
• Coordinate scheduling with Therapy Coordinator and other staff.
• Participate in discharge planning and staff development.
