Angels of Care Pediatric Home Health in Bluffton, SC is hiring Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide one-on-one, in-home care for medically fragile children and young adults. Responsibilities include performing patient assessments, administering medications and therapies, coordinating with families and care teams, and educating families to improve outcomes.

Qualifications

• Active, unrestricted RN or LPN license (SC).
• License acquired 1 year prior to application.
• Ability to provide care in a home setting.
• Strong communication and clinical decision-making skills.
• Pediatric or home health experience preferred.
• Experience with high-acuity skills (trach, vent, G-tube) preferred.

Responsibilities

• Provide skilled nursing care in a home health setting
• Perform ongoing patient assessments and clinical evaluations
• Administer prescribed medications, treatments, and therapies
• Deliver care in accordance with physician orders and plan of care
• Coordinate care with families, providers, and care teams
• Educate patients and family members to support improved outcomes
• Implement preventive measures to ensure patient safety and quality care
• Respond appropriately to changes in patient condition
