Angels of Care Pediatric Home Health is seeking Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) for one-on-one in-home care of medically fragile children and young adults in Sumter, SC and surrounding areas. We offer full-time, part-time, and PRN opportunities with day, night, and weekend shifts.

Qualifications

• Active, unrestricted RN or LPN license (South Carolina)
• License current for at least 1 year prior to application
• Ability to provide care in a home setting
• Strong communication and clinical decision-making skills

Responsibilities

• Provide skilled nursing care in a home health setting
• Perform ongoing patient assessments and clinical evaluations
• Administer prescribed medications, treatments, and therapies
• Deliver care in accordance with physician orders and plan of care
• Coordinate care with families, providers, and care teams
• Educate patients and family members to support improved outcomes
• Implement preventive measures to ensure patient safety and quality care
• Respond appropriately to changes in patient condition
