Job summary

Apex Rehab Group in Ulster County, NY, is seeking Licensed Occupational Therapists and Physical Therapists to join our home care team. If you enjoy one-on-one patient care, independence, and a flexible schedule, this part-time/per diem role could be a great fit.

You will provide therapy in patients’ homes, create personalized treatment plans, track progress, educate families, and complete timely documentation, with support from our clinical team.

Qualifications

• Licensed OT, PT, or ST in New York.
• Home care experience is a plus, but not required.
• Reliable transportation and a valid driver’s license.
• Good communication skills and a positive attitude.
• Ability to manage your own schedule.

Responsibilities

• Provide therapy services to patients in their homes
• Create and carry out personalized treatment plans
• Track patient progress and adjust care as needed
• Educate patients and families
• Complete simple, timely documentation
• Work independently with support from our clinical team
