Job summary

Responsibilities

• Conduct in-home assessments and functional evaluations.
• Deliver personalized, evidence-based recommendations for home modifications.
• Educate clients and caregivers on fall prevention and safe mobility techniques.
• Engage clients and families in collaborative care planning.
• Work closely with the Director of Clinical Operations to share findings.
• Maintain accurate and timely documentation through electronic systems.
• Receive compensation per completed evaluation, including travel.
