Qualifications

Responsibilities

• Complete in-home OT evaluations and deliver skilled visits.
• Build and execute clear care plans focusing on patient independence.
• Recommend and train on adaptive equipment and modifications.
• Coordinate with nursing and the care team as needed.
• Document promptly and thoroughly in Homecare Homebase.
