Job summary

FOX Rehabilitation is seeking licensed Occupational Therapists to deliver one-on-one care to older adults in private residences and senior living communities in Milton, GA. You’ll conduct evaluations, create individualized plans, and deliver evidence-based treatment as part of the Geriatric House Calls™ model.

You will enjoy autonomy over your schedule, the flexibility of in-home visits, and the option to integrate telehealth to extend access and continuity of care.

Qualifications

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Occupational therapy to older adults in home or community settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, longevity, mobility, and independence.
• Document effectively in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access.
