Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to join our Acworth, GA team, delivering one-on-one care in private residences and senior living communities. Our Geriatric House Calls™ model supports autonomy, flexible scheduling, and meaningful patient outcomes, with optional telehealth to extend access.

You’ll collaborate with an interdisciplinary team and educate patients and staff to promote safety and independence in aging in place.

Qualifications

• Active Occupational Therapy license in GA or eligibility to obtain
• Degree from an accredited Occupational Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one Occupational therapy to older adults in homes or senior living communities
• Develop individualized plans of care and deliver evidence-based treatment
• Deliver therapy sessions to improve mobility, independence and quality of life
• Document effectively in the EMR per regulatory standards
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers and staff to promote safety and aging in place
• Offer telehealth/virtual care when appropriate to extend access
