Job summary

Adventist Health's home health and hospice services are seeking an Occupational Therapist to plan and conduct treatment segments that restore physical, social, and mental functions. You will co-lead interdisciplinary patient groups and help drive coordinated care in the home setting.

The position requires a Master’s in OT or OTD, active OT licensure, CPR/BLS certification, and a valid driver’s license. Compliance with vaccination policies is part of employment.

Qualifications

• Master's degree in Occupational Therapy or OTD; license required if obtained after Jan 2007
• Occupational Therapy licensure in all applicable states
• CPR/BLS certification and valid driver's license; age 21+

Responsibilities

• Completes evaluations to ensure quality standards and addresses assessment areas that require attention.
• Uses appropriate clinical techniques and tools and establishes individualized treatment plans for patients.
• Provides therapy interventions using appropriate treatment protocols and techniques.
• Establishes individualized care and discharge plans and ensures plans are communicated to appropriate parties.
• Completes comprehensive and accurate clinical documentation within established deadlines.
• Supervises assigned assistants.
• Performs other job-related duties as assigned.
