Job summary

AdventHealth in Florida seeks a licensed Occupational Therapist to coordinate care for home-based patients, perform functional assessments, and develop patient-specific plans. You will supervise/educate the care team and engage families to support therapy goals.

The role emphasizes safety assessments, documentation, and interdisciplinary collaboration, with a day shift in a Medicare-certified home health setting. OT licensure and DL are required.

Qualifications

• Ability to delegate tasks to appropriate personnel as indicated by skill level and professional standing.
• Knowledge of Home Care Regulations and Third-Party Reimbursement impacting care delivery.
• Demonstrated knowledge of occupational therapy theory, modalities, treatments, and skills.
• Basic computer skills and knowledge of Microsoft Office.

Responsibilities

• Coordinates and directs the care of home patients requiring occupational therapy, including assessment, planning, implementation and evaluation.
• Performs evaluations to assess patients' functional abilities and documents results.
• Develops patient-specific care plans with measurable goals in consultation with patients, families and physicians.
• Implements care plans through direct patient care and coordination with the healthcare team.
• Educates patients and caregivers on therapy techniques and home programs.
