Job summary

UChicago Medicine is seeking an Occupational Therapist for Full-Time Days to join Home Care. The OT will implement standards of care for occupational therapy services and ensure adherence to agency policies.

Key responsibilities include evaluating functional status, developing Plans of Care, teaching patients, and training families in energy conservation and adaptive techniques. Travel to patient homes is required.

Qualifications

• BS in Occupational Therapy from an accredited institution.
• IL License or eligibility. Letter to sit for the National Exam.
• Basic computer proficiency.
• Effective verbal and written communication.
• Current CPR is required.
• Possesses and maintains a valid driver’s license and transportation for home visits.

Responsibilities

• Evaluates patient’s functional status and home environment for barriers.
• Develops a treatment program and Plan of Care; communicates with physicians.
• Teaches new skills and retrains patients in ADLs and IADLs.
• Instructs on use of adaptive equipment and prosthetic/orthotic devices.
• Maintains clinical records and reports changes to physicians.
