Job summary

Adventist Health seeks an Occupational Therapist to join our home health/hospice teams. The role centers on evaluating and treating patients to restore physical, social and mental functions within an interdisciplinary framework.

You will co-lead patient groups, craft individualized treatment and discharge plans, and ensure timely documentation. Candidates must hold a Master’s degree (or OTD) in OT, current licensure in applicable states, CPR/BLS certification, and a valid driver’s license aged

Qualifications

• Master's degree (or OTD) in OT; license on/after Jan 2007 or foreign equivalent.
• OT licensure in all applicable states; CPR or BLS certification; valid driver's license; 21+ years old.

Responsibilities

• Completes evaluations and addresses areas needing attention.
• Creates individualized treatment plans and uses appropriate clinical techniques.
• Provides therapy interventions using established protocols/techniques.
• Develops care and discharge plans and communicates them to relevant parties.
• Completes comprehensive and accurate clinical documentation within deadlines.
• Supervises assigned assistants.
• Performs other job-related duties as assigned.
