Job summary

Hometown Home Health is seeking a licensed Occupational Therapist to provide in-home therapy services throughout the Jacksonville area. You’ll work independently, build strong patient relationships, and help patients regain skills for daily living in their own homes.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Florida OT license required.
• CPR certification is required (online-only not accepted).
• At least 1 year of clinical or institutional experience.
• AOTA membership preferred.

Responsibilities

• Complete occupational therapy evaluations and reassessments in patients' homes.
• Develop individualized treatment plans based on functional needs and physician orders.
• Provide interventions focused on mobility, self-care, safety, and ADLs.
• Educate patients and caregivers on exercises, adaptive strategies, and safe techniques.
• Collaborate with physicians and home health team.
• Monitor progress and adjust plan of care as needed.
• Complete visit documentation and progress notes accurately and on time.
