Job summary

University Home Care in Michigan is seeking a licensed Occupational Therapist to provide skilled home health services. The therapist evaluates functional abilities, develops individualized treatment plans, and helps patients improve safety and participation in daily living activities.

You will travel to patient homes, collaborate with physicians and nurses, document progress, and adapt plans to progress. A valid state license, NBCOT (where required), and reliable transportation are essential.

Qualifications

• Licensed Occupational Therapist in the state of practice.
• NBCOT certification as required by state.
• Valid driver’s license and reliable transportation.
• Ability to travel to patient homes within assigned territory.

Responsibilities

• Conduct occupational therapy evaluations in the home setting.
• Develop individualized treatment plans based on patient needs and goals.
• Provide interventions to improve independence with ADLs and IADLs.
• Assess home environments for safety risks and recommend modifications.
• Train patients and caregivers in adaptive techniques and safe mobility.
• Document evaluations, progress notes, and discharge summaries.
• Collaborate with physicians, nurses, PTs and other care team members.
• Reassess and adjust treatment plans based on progress.
• Educate patients and families on fall prevention and maintenance.
