Qualifications

• Active California OT license is required.
• CPR or BLS certification is required.
• Driver's license and auto insurance are required.
• Physical and TB tests within the last year are required.
• Professional liability and business license (if applicable).

Responsibilities

• Perform in-home assessments for ADLs, IADLs, and home safety.
• Create personalized treatment plans focusing on energy conservation and adaptive equipment.
• Schedule and conduct patient home visits based on your caseload and availability.
• Work with a supportive team to ensure holistic care across the plan of care.
