Job summary

Qualifications

• Active, unencumbered OT license.
• 2+ years of clinical experience in relevant fields.
• Reliable transportation and valid driver’s license.

Responsibilities

• Conduct in-home assessments and evaluations.
• Deliver evidence-based recommendations for home modifications.
• Educate clients on fall prevention and mobility techniques.
• Collaborate with clients and families in care planning.
• Share findings with the Director of Clinical Operations.
