Job summary

Jukebox Health is seeking a contracted In-Home Occupational Therapist to assess homes, recommend safe modifications, and educate clients for safer independence. You will perform in-home evaluations, document findings, and collaborate with a remote clinical team.

A valid OT license and 2+ years of experience are preferred; travel to client homes is required. The role offers flexible weekly hours (minimum 5 hours) and a monthly evaluation-based pay structure.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Proficiency with electronic documentation and digital systems.

Responsibilities

• Conduct in-home assessments and functional evaluations.
• Deliver personalized, evidence-based recommendations for home modifications.
• Educate clients and caregivers on fall prevention and safe mobility techniques.
• Engage clients and families in collaborative care planning.
• Work with Director of Clinical Operations to share findings and support process improvement.
• Maintain accurate and timely documentation through our electronic system.
