Job summary

FOX Rehabilitation delivers in-home and community-based occupational therapy for older adults, offering one-on-one care in private residences and senior living sites. Therapists develop plans, evaluate progress, and deliver evidence-based treatment to improve independence and safety.

Qualifications

• Valid Occupational Therapy license in the state of practice or eligibility to obtain license.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Occupational therapy to older adults in home or community settings.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to support wellness, mobility and independence.
• Document patient progress in FOX’s EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
