Advantmed seeks a dedicated Nurse Practitioner to join our full-time provider network in Oregon. You will perform in-home wellness and risk adjustment assessments, ensuring accurate documentation and high-quality, value-based clinical care across diverse populations.

Travel statewide across Oregon is required, with HIPAA compliance and collaboration with the care team to close gaps in quality and outcomes. Strong clinical skills and EMR proficiency are essential.

Qualifications

• Active, unencumbered Nurse Practitioner license in Oregon.
• Minimum 3 years of direct patient care experience (primary care, adult/geriatric preferred).
• EMR proficiency is desired.

Responsibilities

• Perform in-home wellness and risk adjustment assessments.
• Travel to members’ homes for comprehensive health evaluations and annual wellness visits.
• Review medical history, physical exams, vital signs, and medication reconciliation.
