Job summary

Advantmed is seeking a licensed Nurse Practitioner to perform structured, in‑home wellness and risk‑adjustment assessments. This 1099 PRN role offers autonomy with consistent visit availability and strong operational support.

Enjoy flexible weekday scheduling, with no evenings or weekends, and a reliable income stream through ~100 USD per completed assessment plus mileage. Travel at a 55‑mile radius is typical, with onboarding and ongoing training provided.

Qualifications

• Active unencumbered NP license in the applicable state.
• 3+ years of patient care experience preferred.
• Comfortable with EMR systems and independent work.
• In‑home or risk‑adjustment experience preferred.
• Willing to obtain additional licensure if needed (supported by Advantmed).

Responsibilities

• Conduct in‑home Annual Wellness Visits and chronic condition assessments.
• Perform focused histories, exams, vitals, medication reviews, and screenings.
• Screenings: HbA1c, Spirometry, KED, DRE, FIT.
• Educate members on preventive care and chronic disease management.
• Document visits in the EMR to support value‑based care and risk adjustment.
