Job summary

Advantmed seeks a licensed Nurse Practitioner to conduct structured in‑home wellness and risk‑adjustment assessments. This PRN, 1099 role offers reliable, ongoing income with support and autonomy, centered on in‑home visits and standardized visits.

You will perform comprehensive histories, exams, vitals, and screenings, document in the EMR, and collaborate with a dedicated coordinator for scheduling and escalation as needed. Flexible weekdays, no evenings or weekends.

Qualifications

• Active, unencumbered NP license in the applicable state.
• 3+ years of patient care experience preferred.
• Willing to obtain additional NP licensure if needed (supported by Advantmed).

Responsibilities

• Conduct in‑home Annual Wellness Visits and chronic condition assessments.
• Perform focused histories, exams, vitals, medication reviews, and screenings.
• Document visits in the EMR to support value‑based care and risk adjustment.
