The Judge Group seeks experienced Nurse Practitioners to perform in-home health assessments for Medicare Advantage members. Flexible scheduling and weekend availability support meaningful one-on-one patient care.

Requirements include at least 6 months NP experience, AANP/ANCC certification, and state-specific autonomous licensure for Adult-Gerontology and related specialties; telephonic-only backgrounds are not accepted.

Qualifications

• Minimum 6 months NP experience; no new grads.
• National Board Certification: AANP or ANCC.
• Accepted Specialties: Adult Gerontology Primary Care, Adult NP, Family NP, Gerontology NP, Adult Gerontology Acute Care NP (state-dependent).
• Not Accepted: Mental Health, Pediatric, Women’s Health/OBGYN, Psych, Telephonic-only NP backgrounds.
• Autonomous License Required in: CT, FL, IL, MN, NE, NY, SD, VA, VT.
• Acute Care NP approval only in: AK, AZ, CO, DE, DC, HI, ID, IA, KS, KY, ME, MD, MA, MI, MO, MT, NV, NH, NJ, NM, ND, OK, OR, RI, UT, VT, WA, WV, WY.

Responsabilités

• Nurse Practitioners conduct 45–90 minute in-home assessments including medical history, ROS, physical exam, vitals
• Medication review & depression screening
• Identification of diagnoses and care gaps
• Patient education and referral identification
