Hueman is hiring Nurse Practitioners to conduct in-home Health Risk Assessments for Medicare/Medicaid members in Bonner County, Idaho. The role supports patients in their homes, away from clinics, with a focus on preventive care and wellness education.

Qualifications

• Masters Degree OR commensurate experience and NP license completion.
• Active State NP license.
• Active National board certification.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider is preferred but not required.

Responsibilities

• Travel to patient homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.
