Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members around Crawford, GA. This role offers flexible scheduling and travel to patients' homes, with comprehensive training and scheduling assistance provided.

Nurse Practitioners will not treat or prescribe in the home, focusing on assessments and patient interaction in non-clinic settings.

Qualifications

• Master’s degree or commensurate experience and satisfactory NP license.
• Active State NP license required.
• Active national board certification required.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider preferred.

Responsibilities

• Travel to and from patients’ homes to perform HRAs.
• Conduct comprehensive, non-invasive physical assessments and review medical history.
• Document findings accurately and perform medical reconciliation for patient safety.
• Provide basic patient education on wellness and preventive healthcare.
