Qualifications

• Active and unrestricted RN and NP license, or Physician Assistant license.
• Board certification (NP: ANCC/AANP | PA: NCCPA).
• Current and unrestricted DEA certificate, or eligibility to obtain.
• Minimum 2 years of direct patient care experience.
• Travel to patient homes with reliable transportation.
• BLS for Healthcare Providers is required.
• Knowledge of HIPAA/OSHA regulations.
• Experience with EHR systems (Athena, Salesforce) preferred.

Responsibilities

• Provide evidence‑based, patient‑focused home-based care.
• Conduct comprehensive initial evaluations and wellness exams.
• Educate patients and caregivers on disease management and medications.
• Participate in virtual integrated care team meetings.
• Coordinate care with PCPs, specialists, and ancillary services.
• Complete documentation accurately and timely.
• Periodic travel to patient homes may be required.
