Job summary

Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations for health plan members in their homes. You will perform medication and medical history review, a physical evaluation, and additional tests when ordered by the health plan, using an iPad-based workflow.

You will join a national network of clinicians, travel locally, and participate in virtual visits as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).

Responsibilities

• Provide In‑Home Health Evaluations for health plan members.
• Perform medication and medical history review, physical evaluation and related tests as ordered by health plan.
• Travel locally within licensed states; conduct virtual visits as needed.
