Job summary

Optum seeks a Part-time Advanced Practice Clinician (NP/PA) to perform in-home health assessments across Clallam County, WA, for Medicare Advantage and other health plan members. This field-based role carries no prescribing responsibilities, emphasizing preventive care and care coordination.

You will work with PCPs and interdisciplinary teams to document diagnoses, address gaps in care, and educate patients on conditions, medications, and adherence.

Qualifications

• NPs: ANCC or AANP certification in Family, Adult, Gerontology or Primary Care.
• PAs: NCCPA certification and state licensure.
• Active, unrestricted state license; valid driver’s license and reliable transportation.

Responsibilities

• Conduct in-home health assessments, including medical history review, physical exams, and vital signs.
• Perform medication reconciliation and preventive screenings.
• Document diagnoses to support care management and communicate findings to PCPs.
• Identify gaps in care and connect members with resources; educate on conditions and medications.
• Assess social determinants of health and provide referrals as needed.
