Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations in members’ homes. You will conduct comprehensive assessments, review medications and medical history, and coordinate with health plans to connect members with the right care.

This role offers flexibility to travel locally and within licensed states, remote-visit options as needed, and a typical 29.5 hours per week.

Qualifikationen

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multistate licenses preferred.
