Job summary

CVS Health's Signify Health seeks a Part-time Clinician (NP/PA) to provide in‑home health evaluations across participating states.

You’ll travel locally, conduct visits lasting about 45 minutes, perform med history reviews and physical evaluations, and deliver Diagnostic & Preventive Services as indicated by the health plan. As part of a national network, you’ll connect members with the right care and use an iPad workflow.

Qualifications

• Active, unrestricted licenses in coverage areas.
• Board certification is required.
• Multi-state licenses preferred; able to cover State College area.

Responsibilities

• Provide in-home health evaluations to members.
• Build trusted relationships to improve health outcomes.
• Travel locally within licensed states as needed.
• Perform virtual visits when required.
• Deliver Diagnostic & Preventive Services as indicated by plans.
