• Provide In-Home Health Evaluations for health plan members in their homes
• Engage with patients to review medications and medical history
• Perform physical evaluations and collect health data
• Travel locally within licensed states; conduct virtual visits when needed
• Use Signify Health tools and workflows to document visits accurately
