Job summary

Signify Health, part of CVS Health, is seeking a Full-time Clinician (NP or PA) to perform in‑home health evaluations in the Springfield, MA area. You will conduct comprehensive assessments in patients' homes, review medical history, and perform physical evaluations using a streamlined digital workflow.

The role emphasizes patient relationships, flexibility to travel within licensed states, and providing quality, personalized care without prescribing or ordering labs.

Qualifications

• Active, unrestricted NP/PA license in the coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Conduct a 45‑minute in-home health evaluation; connect with patient and family.
• Review medication history and perform medical history review and physical evaluation.
• Use iPad and clinical workflow; no prescribing, no lab orders, or treatment changes required.
• Travel locally to Springfield, MA – Hampden County; virtual visits as needed.
