Signify Health is seeking a Part Time Clinician (NP or PA) to perform in-home health evaluations for members across licensed states. You will visit patients in their homes, review medical history, perform a clinical evaluation, and connect them to appropriate care.

Typical week is about 29.5 hours, with virtual visits as needed. The role uses an iPad and a streamlined workflow; no prescribing, lab orders, or changes to current treatment unless directed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.
