Job summary

Signify Health, part of CVS Health, seeks a Part Time Clinician (NP or PA) to perform In-Home Health Evaluations, visiting patients in their homes across Maine, starting with Portland and surrounding areas. You’ll help bridge gaps in care with a patient-centered approach and document findings using an iPad workflow.

The role emphasizes compassionate patient interaction, ability to travel locally, and delivering comprehensive assessments without altering current treatments.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.
• Able to cover Portland, ME and surrounding area.
