Job summary

Signify Health, part of CVS Health, is seeking a Part Time Clinician (NP or PA) to provide in-home health evaluations, visiting members in their homes and helping bridge gaps in care.

You will join a national network of clinicians, offering flexible visits and comprehensive evaluations, with local travel and virtual visits as needed. Board-certified, licensed clinicians preferred, with multi-state licenses.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations to assess and bridge gaps in care.
• Travel locally within licensed states; conduct virtual visits as needed.
• Operate using iPad and clinical workflow; no prescribing or lab orders required.
• Deliver diagnostic and preventive services as indicated.
