Job summary

Signify Health is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform in‑home health evaluations, reviewing medications and medical history, conducting physicals and optional tests. The role uses an iPad and a streamlined workflow, with no prescribing or lab work required.

The position involves traveling locally and within licensed states, performing virtual visits as needed, and delivering comprehensive diagnostic and preventive services to plan members.

Qualifications

• Active, unrestricted license in the coverage area.
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Provide in-home health evaluations for patients in their residence.
• Perform medication and medical history reviews, physical evaluations and needed diagnostics.
• Utilize iPad and clinical workflow; no prescribing or lab orders required.
• Travel locally and within licensed states; conduct virtual visits when needed.
• Coordinate with Signify Health services including diagnostic and preventive testing.
