Collage Home and Community Rehabilitation Partners seeks experienced Licensed Clinical Social Workers to provide patient-centered care in Perris, CA and surrounding areas with PRN positions for clinicians who will accept patients for the duration of their stay.

Qualifications

• Master's degree in Social Work and state licensure as an LCSW are required
• Excellent planning, organization, writing, and time management skills are required

Responsibilities

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
