Job summary

Collage Home and Community Rehabilitation Partners seeks an experienced Occupational Therapist to deliver patient-centered care in Escondido, CA and surrounding communities. PRN opportunities are available to support home-based neurological rehabilitation programs.

You will assess and treat impairments, develop individualized plans, and collaborate with families and the care team to maximize functional outcomes in patients’ homes and communities.

Qualifications

• Bachelor's degree in Occupational Therapy with state licensure required.
• Master's degree in OT preferred.
• Experience with neurologically related disorders preferred.
• Strong planning, organizational, writing and time-management skills required.
• Ability to communicate with families, professionals and co-workers is essential.

Responsibilities

• Evaluation and treatment of impairments including upper extremity function, ADLs and IADLs.
• Address visual and perceptual skills, cognitive needs, and adaptive equipment needs.
• Involve assistive technology needs and functional goals in home settings.
