Job summary

Collage Home and Community Rehabilitation Partners seeks an experienced Occupational Therapist to provide in-home, patient-centered care in Charlotte, NC and surrounding areas. Qualifications

• Bachelor's degree in Occupational Therapy is required and licensure must be current.
• Neurologically related disorders experience preferred.
• Excellent planning, organization, writing and time management skills required.
• Ability to communicate with families, professionals and co-workers.

Responsibilities

• Evaluate and treat impairments including upper extremity function, ADLs and IADLs, cognitive needs, adaptive equipment needs, assistive technology needs.
• Deliver patient-centered home-based OT services in Charlotte, NC and surrounding areas.
• Collaborate with interdisciplinary team to develop and implement integrated treatment plans.
