Innovative Integrated Health Inc in Anaheim, CA is seeking a licensed vocational nurse (LVN) to support home care, clinic, and adult day health services under supervision. You will perform in-home nursing tasks, administer medications, and contribute to care plans while coordinating with the IDT.

This role requires strong patient communication, adherence to California LVN scope, and thorough documentation in the electronic health record system during home visits.

Qualifications

• Active LVN license in California and CPR certification.
• Experience in delivering in-home nursing care and patient safety.
• Ability to follow care plans and document in EHR.

Responsibilities

• Participate in IDT and Care Plan Meetings as scheduled.
• Administer medications and provide education to patient/family under supervision.
• Perform vital sign measurements and basic wound care during visits.
• Assist with rehabilitative exercises and activities as prescribed.
