Job summary

Doctor's Choice Home Care & Hospice Texas is seeking a Licensed Vocational Nurse (LVN) to provide one-on-one skilled nursing care in patients' homes on an intermittent basis. The LVN will collaborate with physicians, families, and the care team to implement and monitor patient-specific plans under RN direction.

The role requires current Texas LVN licensure, CPR certification, reliable transportation, and willingness to travel up to 100% to patient locations.

Qualifications

• Minimum of one year nursing experience (preferred).
• CPR certification with American Heart Association or American Red Cross.
• Ability to exercise initiative and independent judgment.
• Strong written and oral communication skills with excellent interpersonal abilities.
• Commitment to quality customer service.
• May be on call at least once per month.
• Valid driver's license, auto liability insurance and reliable transportation.
• Willingness to support agency values and goals.
• On-call may be required and 100% travel to patient homes.

Responsibilities

• Administer one-on-one skilled nursing care in patients' homes.
• Collaborate with physicians, families, nurses, therapists and social workers.
• Implement and monitor patient care plans under RN direction.
• Assess, document and report patient progress using clinical notes.
• Adhere to agency policies while traveling to patient locations.
