Matrix Home Health Care is seeking a Licensed Practice Nurse to provide skilled nursing services in the home setting across Florida. The contractor will deliver care aligned with the patient’s plan of care, adhere to nursing standards, and ensure compliance with federal and state regulations.

Responsibilities include direct patient care, accurate documentation, patient/family education, and coordination with the clinical team to promote patient wellbeing and safe home health delivery.

Qualifications

• Licensed Practice Nurse in state of practice in good standing.
• At least one year of nursing experience; home health experience preferred.
• Ability to interpret instructions and communicate with patients and staff.
• Knowledge of home health care regulations and safety standards.

Responsabilités

• Provide direct and individualized nursing care to home care patients per plan of care.
• Document services accurately and timely in patient records.
• Educate patients and families about health conditions and self-care.
• Coordinate home health services with the patient’s clinical management team.
