Job summary

Interim HealthCare Home Health is seeking a Licensed Practical Nurse (LPN) Visit Nurse to provide in-home care across Marion and Marysville, OH. You will perform assessments, wound care, and patient education according to care plans, delivering compassionate, patient-centered service.

The role emphasizes high-quality, continuous care with flexible scheduling and a per-visit pay structure. Home health experience is preferred and reliable transportation is required.

Qualifications

• Graduation from an accredited practical/vocational nursing program.
• Current, unrestricted LPN license in the state of Ohio.
• Current CPR/AED/BLS/First Aid certification.
• Nursing experience in home health or similar setting preferred.

Responsibilities

• Provide in-home direct patient care per plan of care and nursing scope.
• Complete skilled nursing assessments and patient-centered care in the home.
• Perform wound care and monitor wounds for changes or complications.
• Evaluate patient needs and implement the appropriate plan of care.
• Communicate changes in patient conditions to the care team for continuity of care.
• Complete clinical documentation per agency protocols and Medicare guidelines.
• Educate patients and families involved in care.
• Help decrease readmissions by promoting preventative care.
