Qualifications

Responsibilities

• Provide direct one-on-one nursing care for an adult patient in the home.
• Monitor and assess the patient's condition and report changes.
• Administer prescribed medications, treatments, and therapies.
• Follow physician orders and individualized care plans.
• Assist with disease management and health maintenance.
• Maintain accurate and timely nursing documentation.
• Educate and support patients and family members as needed.
• Ensure patient safety and promote optimal health outcomes.
