Job summary

Maxim Healthcare in La Junta, CO is hiring a Licensed Practical Nurse (LPN) to care for adult patients in their own homes. This full-time role offers flexibility with part-time and PRN options in La Junta, Ordway, Las Animas and Lamar areas.

The LPN will deliver skilled nursing care using the nursing process, assess patient needs, and document findings in coordination with the care team. A valid LPN/LVN license and current BLS are required.

Qualifications

• Currently licensed as an LPN/LVN in the state where the LPN will practice.
• Current TB or Chest X-Ray.
• Current BLS card.
• One year prior professional nursing experience preferred

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• On each assignment, provides skilled nursing care/services in accordance with prescribed orders
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
